 Reporting a Death  Often, an active member’s employer will notify the Retirement Systems of the member’s death; however, it is always a good idea to contact the Retirement Systems upon the death of a member whether the member is active or retired. You may notify us initially by e-mail, by telephone at (800) 868-9002 (within SC only) or at (803) 737-6800, or in writing at PO Box 11960, Columbia, SC 29211-1960. Please be prepared to provide the member’s name, Social Security number, date of death, and the appropriate contact name, address, and phone number. Upon notification of a death, the Retirement Systems will send the necessary forms to you for completion. Please remember to include all documentation requested when you file your claim.
