Reporting a Death

Often, an active member’s employer will notify the Retirement Systems of the member’s death; however, it is always a good idea to contact the Retirement Systems upon the death of a member whether the member is active or retired.

You may notify us initially by contacting Customer Services at 803-737-6800, toll free at 800-868-9002 (within SC only), using our online contact form, or notify us in writing at P. O. Box 11960, Columbia, SC 29211-1960. Please be prepared to provide the member’s name, Social Security number, date of death, and the appropriate contact name, address, and phone number.

After receiving the notification, the Retirement Systems will send the necessary forms to the member's designated beneficiary or other designated individual, such as a personal representative or power of attorney, for completion. Please remember to include all documentation requested when filing a claim for death benefits.
